Personal details provided with the submission of this form are acquired solely for the performance of activities of assistance requests by users. For this purpose the data will be collected, recorded and processed in an automated way with procedures strictly necessary for pursuing the object. The manager and the treatment will therefore become aware of the data provided. Data will not be disseminated or disclosed to third parties and can be accessed simply by writing addressed to the data controller to verify the use or to correct, update, cancel or oppose to their treatment in accordance with art. 7 of this Act.
